In accordance with North Dakota University System policy, the University of North Dakota's Student Health Services (SHS) oversees and executes immunization compliance for the UND campus.
All students enrolled at UND must provide documentation of required immunizations, or request an approved exemption.
Failure to comply with the immunization requirements will result in a hold on your account, restricting you from registering for classes for the following semester.
Once you have claimed your UND account, you may access your MyHealth Patient Portal to enter your immunizations and upload immunization information electronically. Use your UND username and password to log in.
If you are unable to enter and upload your immunization information online, you can fax your immunization records to 701.777.4835.
Students enrolled solely in distance learning courses; immunization is contraindicated by a medical condition; or the student's belief precludes participation in an immunization program may request an exemption.
To request an exemption, log into the UND MyHealth Patient Portal and complete the SHS Immunization Exemption Request Form or print Immunization Exemption Request Form and submit to Student Health Services for approval.
Requesting an exemption does carry risk. In the event of an outbreak of a communicable disease in which immunization is required (see above for required immunizations), Student Health Services staff and/or the local public health officer will determine exclusions from campus and campus activities until the danger of the epidemic is over.
